Forms let you collect customer details and issue loyalty cards instantly. You can enable default fields like name and email, and add your own custom fields (text, date, dropdown, single choice and multiple choice.) to collect any extra data you need — all while matching your brand's style.


Overview

Each form is tied to a loyalty card and can be customized with both visual and data fields. Forms are typically used to:

  • Enroll new customers

  • Assign them a specific card

  • Collect birthdays, preferences, or survey data

  • Get GDPR-compliant marketing consent

  • Display a success screen with a digital card link

Each form has two views: the Form Preview (customer-facing form) and the Success Preview (what customers see after joining).

Forms can be shared via links, embedded on your website, or printed as QR code flyers.


How It Works

Step 1: Customer fills the form

They enter their information — including optional custom fields you’ve configured.

Step 2: Card is issued

Once submitted, the assigned card is issued immediately and shown on the success screen.

Step 3: Wallet-ready

The customer can then add the card to Apple Wallet, Google Wallet, or save the link.

You can display this message in-store via QR flyer or embed the form on your website to onboard new members seamlessly.


Creating a Form in Loyaltify

1. Select Card

Choose the loyalty card that will be issued to the user upon form submission.

2. Form Details

  • Form Description: Shown at the top of the form. You can add emojis, and bold text.

  • Submit Button Text: Customize to say things like “Join now”, “Get my card”, etc.

  • Background/Text Color: Match your brand aesthetic.

  • Unique Field: Choose either Email or Phone to avoid duplicate customers.

3. Default Fields

Toggle each one to enable and/or require:

4. Custom Fields

You can add as many custom fields as you want. Here are supported types and example uses:

Text

  • Label: How did you find us?

  • Collects short open-ended responses (e.g., "Instagram", "Friend")


Date

  • Label: Anniversary Date

  • Useful for relationship-based campaigns or special date reminders


Dropdown

  • Label: Gender

  • Options: Male, Female, Non-binary, Prefer not to say

  • Good for standardized choices


Single Choice

  • Label: How often do you visit us?

  • Options: First time, Daily, Weekly, Monthly

  • Only one option selectable


Multiple Choice

  • Label: What do you enjoy at our cafe?

  • Options: Coffee, Tea, Pastries, Wi-Fi

  • Allows selecting multiple answers

5. Success Preview

After submitting, users see:

  • A confirmation message

  • A "Save to Wallet" option (Apple Wallet or Google Wallet)


Final Step: Manage Forms

Click the three-dots menu (⋮) on any form to manage it:

  • Edit Form: Update text, form, or styling

  • Delete Form: Permanently remove

  • Copy URL: Get the public link to share via email, SMS, or web

  • Download QR Code (SVG/PNG): Save a QR code to print on flyers or signage for easy access
